CityCare Connect - Nottingham

CityCare Connect is a short term re-ablement facility that operates as a stepping stone between hospital and home. Residents at Connect House stay for a maximum of four weeks, because that’s all our residents need before they are happy, healthy, confident and ready to regain their independence at home.

CityCare Connect Staff

From complex needs, to simple needs, residents in Connect House are cared for by the full range of medically trained professionals. Nurses, pharmacists, physiotherapists and occupational therapists are in attendance daily, in addition to any specialist staff as required by our residents.

Health care professionals attending CityCare Connect are provided by CityCare, a leading community health care provider and has vast experience in providing a full range of services to the people of Nottingham and beyond.

Our staff are carefully selected to ensure they are right for our residents. Appropriate qualifications are the basic requirement, it is essential they be caring and compassionate, and they must display the CityCare values of Integrity, expertise, unity and enterprise.

Gaynor holds recognised qualifications as required by the Care Quality Commission to be the Registered Manager at Connect House. Gaynor has over 30 years’ experience in a variety of Health and Social Care settings.

Carol has spent her entire career in healthcare and has board-level managerial experience in public, private and not-for-profit organisations in the UK and overseas. Carol is still a practicing pharmacist so brings a mix of professional and managerial skills to the Board.

Tracy brings a long record of service in the health sector and is a Registered Nurse and holds an MSC in Public Sector Management. Tracy has worked in a number of healthcare settings in both provider and commissioner organisations undertaking a wide range of clinical and leadership roles. Tracy is passionate about driving forward high quality patient safe care that provides our patients with the highest experience through integrated working across the health and social care system and ensuring both innovation and opportunities are maximized.

Loraine has over 25 years nursing experience, 18 years of which has been community based, working as a district nurse practice teacher and more recently as a specialist nurse practitioner for care homes. As Lead nurse Loraine is committed in ensuring that residents receive high quality person centred, compassionate care. Caring for both residents and their relatives in a relaxed friendly environment.

Phyllis Brackenbury - Director of Operations and Transformation

Phyllis has extensive clinical experience in nursing, midwifery and health visiting, and has worked in Nottinghamshire for around 30 years. Phyllis holds an Msc in Public Health and has worked in Health and Local Authority settings. She is committed to transforming and improving the services provided to our local population.

CityCare Connect Facilities

Bright, modern and spacious, Connect House offers residents home from home comforts in a safe and secure environment. Set in a peaceful cul-de-sac, ancient oak trees provide shelter from the sun and rain and carefully landscaped grounds provide a pleasing aesthetic and a relaxing, comfortable environment in which to recuperate.

Individual en-suite rooms

Each of CityCare Connect’s rooms feature an en-suite private bathroom, comfortable electronically adjustable bed, ample personal storage space and an entertainment system for those switch off moments.

Reading room

Tucked away off the main lounge, away from the hustle and bustle of daily life is the reading room. With comfortable high backed chairs and lovely views of the garden, it’s a great place to settle down to read the latest best seller or the day’s paper.

On site salon

Everyone likes to look their best and residents at CityCare Connect are no exception. Our fully equipped salon can cater for everyone regardless of mobility issues, all the while under the watchful care of Connect staff.

Dining and activity area

Our spacious dining area looks out onto a private terrace surrounded by a delightful garden. The dining area has ample space for a comfortable dining experience provided by our onsite chef, and once dinner is done there is ample space for dancing.

“My first visit to see my uncle. The home looks excellent and he looked very happy in the communal area. He was very proud of “his room”. The staff appear to be very attentive to each individual need which is very comforting. The staff were also very helpful when I arrived and did take care of what I asked for in relation to my uncle’s needs. Excellent, keep up the good work.”

Connect House is a reablement facility. Residents are transferred into our care from the hospitals in Nottingham once they are identified as being medically fit, but may not yet be physically able, to support themselves at home. There is no ‘average’ service user at Connect House and we offer very much more than the average rehabilitation or convalescence service that people may imagine.

We work closely with CityCare services and other organisations providing community support to develop a ‘wrap around’ service for even the most complicated and varied needs. We have access to clinical services within Connect House from CityCare teams including physiotherapy, occupational therapy and nurses, as well as support from local GPs and pharmacists. As an independent organisation we can also call on any other appropriate services to benefit the people in our care.
